WHAT TO EXPECT DURING YOUR FIRST VISIT

The first session will consist of a 45 minute to 1 hour evaluation with one of our highly skilled Doctors of Physical Therapy. We will assess your range of motion, strength, flexibility, posture, gait, balance and coordination. We will then develop a treatment plan that fits your needs.

WHAT TO BRING WITH YOU

• Driver’s License

• Prescription for Physical Therapy (from Primary Physician or treating Physician)

• Insurance Card

For those receiving services through Worker’s Compensation or Auto Insurance:

• Claim Number

• Adjuster’s name and phone number

• Case manager’s name and phone number

MAJOR MEDICAL INSURANCE

Our staff will verify your medical insurance benefits for Physical Therapy prior to your first visit and review them with you at your first visit.  If you have an insurance that we do not accept, we do offer a competitive self-pay rate.  We can provide payment plans for those that require financial assistance.  Please contact our office for more information.

We accept cash, check, or credit cards for copays, deductibles, and self payments.
