WHAT TO EXPECT DURING YOUR FIRST VISIT

The first session will consist of a 45 minute to 1 hour evaluation with one of our highly skilled Doctors of Physical Therapy. They will assess your child’s age appropriate milestones, range of motion, strength, flexibility, posture, gait, balance and coordination. They will then develop a treatment plan that fits your child’s needs. Children should wear comfortable clothing and socks.

Please note: shoes are not permitted in treatment room.


WHAT TO BRING WITH YOU

  • Driver’s License

  • Prescription for Physical Therapy (from Pediatrician or treating Physician)

  • Insurance Card

For those receiving services through the Department of Education:

  • RSA2 form from the Department of Education (if applicable)

  • Copy of your child’s Individualized Education Plan (IEP) (if applicable)


MAJOR MEDICAL INSURANCE

Our staff will verify your medical insurance benefits for Physical Therapy prior to your first visit and review them with you at your first visit.  If you have an insurance that we do not accept, we do offer a competitive self-pay rate.  We can provide payment plans for those that require financial assistance.  Please contact our office for more information.

We accept cash, check, or credit cards for copays, deductibles, and self payments.
